Soko Nurse Network
A growing, private, referral-built community of nurses that Soko curates. Your opening goes to our own network first, not a job-board posting.
Nurse recruitment · Home and community care
Case managers keep patients moving through the system safely: discharge planning in the hospital, the plan of care in home health, utilization review for a plan. Soko recruits RN case managers onto the employer’s own staff.
For facilities
A case manager opening backs up discharges, delays admissions, or pushes a caseload past what the team can hold. Soko recruits RNs with case management, utilization review, or discharge planning experience, and CCM or ACM certification where you require it.
For nurses
Case management is the most common path off the floor without leaving nursing. Tell us your setting, your certification, and whether you want field, hospital, or remote-friendly work, and we bring roles that fit.
How Soko works
A growing, private, referral-built community of nurses that Soko curates. Your opening goes to our own network first, not a job-board posting.
Soko was founded by a registered nurse. Every candidate is reviewed for units, patient loads, and fit with the role before an introduction.
Every opening and every nurse is captured on the same details: license, setting, units, shift, and location. Requests are scored the moment they arrive, so the strongest matches move first.
Hospitals and health plans often prefer CCM or ACM; home health and hospice case managers usually need OASIS or hospice experience instead. We confirm the requirement for each opening.
Yes, often through a hospital discharge planning or home health case manager role. Tell us your experience and we are honest about which openings are open to that move.
Some payer roles are; hospital and home health roles are not. The form asks where you will work and how far you will commute, and we match on it.
