This policy describes how SOKO HEALTHCARE LLC, doing business as Soko Healthcare, handles information submitted through this website and its inquiry forms.
Information you choose to share
Nursing inquiries request a name, email, optional phone number, employment preference, and a short message. Facility inquiries request a contact name, organization, work email, optional phone, positions being sought, and optional details. General inquiries request a name, email, audience category, and message. Each form also records your privacy acknowledgment and inquiry type.
These are employment and business inquiry forms, not medical intake forms. Do not submit patient information, medical details, Social Security numbers, birth dates, banking information, license numbers or documents, identification documents, résumés, or other highly sensitive data.
How inquiry information may be used
Soko Healthcare may use the information to respond to you, understand your preferences or hiring needs, and discuss appropriate recruiting next steps. An initial inquiry does not automatically authorize publication of your details. Any further recruiting information or introduction process should be discussed directly.
Storage, hosting, and service providers
OpenAI Sites hosts the website, FormSubmit processes and emails inquiry submissions, and GoDaddy-provided Microsoft 365 hosts the business mailbox. FormSubmit sends submitted information to the Soko Healthcare mailbox and states that it retains submissions for 30 days. Microsoft 365 may retain delivered messages and backups under its service practices. The website itself does not save form drafts or maintain an inquiry database.
These providers may process technical information needed to operate, secure, and deliver their services. OpenAI Sites records aggregate traffic information such as page views and unique visitors. Soko Healthcare does not receive raw form contents through its site analytics, and this website does not add separate analytics or session-replay scripts.
Cookies and analytics
This website does not use advertising cookies, client-side analytics, session replay, or third-party font requests. Hosting and form-delivery providers may use necessary technical measures to provide and protect their services. Their own privacy notices describe those practices.
Sharing and access
Soko Healthcare may share information with service providers that operate the website, form delivery, and business email. It may also share information when reasonably necessary to discuss a recruiting introduction, with your involvement or as otherwise permitted by law. Access should be limited to people who need the information for these purposes. Information may also be disclosed when required by law or to protect rights, safety, and security.
Security
The website uses HTTPS, limits input lengths, checks required fields, and uses spam controls. Access to delivered inquiries is protected through the business email account and provider controls. No website or email system can guarantee absolute security.
How long information is kept
Inquiry messages are kept only as long as reasonably needed to respond, support recruiting conversations, meet legal obligations, and resolve disputes. FormSubmit states that form submissions are retained for 30 days. Messages delivered to Microsoft 365 may remain there until deleted under SOKO HEALTHCARE LLC's mailbox practices.
Access, correction, and deletion requests
You may ask about personal information you submitted or request that it be corrected or deleted, subject to applicable legal and operational requirements. Email info@sokohealthcare.com. Only provide enough information to identify the inquiry; do not send identity documents through the website forms.
